ACTION REQUIRED: Diabetes Audit needs to be started on 1st October to satisfy PQS requirements

Please ensure your team is ready to commence its data collection of conversations with people with diabetes

For the next 4 months all community pharmacy teams should be asking diabetes patients whether they have had their foot and eye checks. The outcome of these conversations should then be recorded on the patient’s PMR.
At the end of the 4 months, pharmacies will then be required to submit the numbers and details of conversations asked. The following link provides the necessary recording paperwork to keep track of this during the period.
If patients haven’t had their eye check (retinopathy scan), then signposting information can be found on each 'Locally Commissioned Services & Local Information' section of the CPSC website under the 'Signposting Information & Guidance' pages:
There is a list of retinopathy scan venues and then contact details for making appointment bookings.
