REMINDER: Quit4Life Smoking Cessation Survey Monkey poll

Quit4Life are requesting you to share your views on what could be done to increase the level of interest of community pharmacies in wanting to provide this service. CPSC supports this drive to enable an increased number of pharmacies providing smoking cessation services across Hampshire
This survey is for HAMPSHIRE based community pharmacies ONLY (i.e. not Southampton, Isle of Wight or Portsmouth)
The survey has been designed by Quit4Life in collaboration with CPSC as a consequence of our ongoing discussions to look at ways in which to increase the interest and number of pharmacy contractors approved to be able to provide this commissioned service.
I encourage all Hampshire based pharmacies to complete the survey (only 9 questions; estimated 3 minutes to complete) and review the current service contract which is attached within the survey (will take a bit longer).
If you are interested in adding this commissioned service to the portfolio of services provided from your pharmacy, please enter your contact details into the last question of the survey. Quit4Life are keen to talk to new pharmacies where a service has not previously been offered.
