Documenting in patient clinical records
What you write in the clinical notes section matters.
CPPE e-learning:
Pharmacy professionals, as an integral part of the NHS 10 Year Health Plan for England: Fit for the Future, are now routinely sending patient information back to the GP surgery for inclusion in the patient’s record. The need for effective communication between healthcare professionals working in different sectors is important.
This e-learning programme will support you with how to document in the clinical notes section that will go into the patient’s records to enable continuity of patient care in integrated local services.
