Clexane to Inhixa Switch planned for Isle of Wight

Isle of Wight CCG are recommending that practices use the Inhixa® brand for all new patients initiated on Enoxaparin and switch patients current using Clexane® (or generic Enoxaparin) to Inhixa®

Isle of Wight CCG are aligning the primary care prescribing of enoxaparin with the local and mainland Trusts. They are planning to send this information to the community pharmacies regarding a brand switch from Clexane to Inhixa because:
- The local and mainland Trusts have switched to the Inhixa® brand for enoxaparin to be assured of consistent stock availability.
- To support patient safety by ensuring that patients consistently receive the same brand of enoxaparin the Isle of Wight CCG are recommending that practices use Inhixa® brand for all new initiations and switch the 71 patients current using Clexane® (or generic Enoxaparin) to Inhixa®. This clinical advice has been approved by PCPC and IMOC.
As patients need to be informed in person about the change of brand and the second click (bullet point 9 on the patient information leaflet) required to activate the protective needle cover, so the Medicines Optimisation Team will not be switching patients remotely.
Practices will need to speak to each patient to ensure that they know the brand they are using and understand the differences. Pharmacies may wish to liaise with their local practice to support this so that they can manage their stock levels, provide information leaflets and support patients with any queries.
Pharmacies should expect an increase in the number of prescriptions for Inhixa®. The gradual switch will enable pharmacies to manage their stock levels of Clexane® and Inhixa®. It is anticipated that a small number of patients may wish to remain using Clexane®. What is important, is that all Enoxaparin is prescribed by brand.
If a pharmacy receives a prescription for Enoxaparin, then please check with the patient what brand they had previously or on discharge from hospital.
The Inhixa® patient instruction tear-off pads are being posted to every general practice and pharmacy.
