Freedom to Speak Up Guardian

Needed for Community Pharmacy Assurance Framework (CPAF)

The Community Pharmacy Assurance Framework requires that Community Pharmacies have appropriate procedures in place for raising concerns, which aligns with NHS-wide expectations for speaking up.
Pharmacy contractors are expected to name an individual as the Freedom to Speak Up Guardian so staff know who to contact if they have a concern; this person must be independent of management within the pharmacy and not the direct employer.
Community Pharmacy Hampshire & Isle of Wight on behalf of HIOW Local Pharmaceutical Committee is providing all contractors with a Freedom to Speak up Guardian, if required.
Please check with your head office before completing the attached agreement to make sure someone has not already been selected within the company.
If you wish to use the LPC Freedom to Speak up Guardian, complete the agreement and email to: artur.pysz@cpsc.org.uk.
