Patient Shared Care Record Access
Community Pharmacists in Frimley ICS and surrounding area can now have access to Connected Care.
The Connected Care Shared Care Record provides Health and Social information about residents including items such as:
- GP Medications, Problems & Allergies,
- Recent Hospital Admissions
- Packages of Care for Mental Health and Social Care.
