Health Innovation Network Programme launch

We are delighted to announce the re launch of the Health Innovation Network “Preventing Problematic Polypharmacy Programme" supporting safer prescribing in older people and those with frailty.
The programme will be led by Amy Semple and Clare Howard and delivered through a cluster approach between Health Innovation Wessex, Health Innovation West of England and Health Innovation East Midlands.
It aims to build on the success of the previous polypharmacy programme and the focus for 26/27 will be:
- To reduce the number of people aged 75 and over taking 10 or more medicines to reduce potential harm and unnecessary healthcare utilisation.
- To widen the scope of the programme to focus on frailty, AI, Digital(NHS App/innovations), repeat prescribing, community pharmacy and secondary care.
- To create a sustainable legacy programme via NHSBSA, NHSE Training Hubs, NHS App/primary care IT systems.
- To evolve the existing HIN national polypharmacy shared learning network into a National Deprescribing Network for England.
