This document has been developed to support GP practices and Community Pharmacies to work together to monitor blood pressure in patients with hypertension.

It includes a template process, text (SMS) wording for patients, and a sample letter to support engagement with local Community Pharmacies.

The Hypertension case finding service is a great way to improve the regular monitoring of patients with hypertension, saving appointment times and offering an alternative and often more convenient option for patients.

Surgeries can find out how many patients in their practice have not had a blood pressure reading within the last 12 months via CVD Prevent data here: HIOW CVDPREVENT | Tableau Public, or identify a list of patients using the clinical system searches suggested within this guide.
