REMINDER: Direct Oral Anticoagulant (DOAC) Survey

Last call for your help in completing this important survey will be much appreciated
Hampshire Hospital NHS Trust re-started their project on DOAC counselling following instigation of this treatment back in July.
They are seeking your perspective about your involvement in counselling patients newly started on anticoagulation medication and/or discharged from hospital for an anticoagulation related issue.

This has the potential to impact upon patient care and empower patients to better manage their medicines thereby reducing the incidence of bleeding related side effects.
