A useful resource to ensure the whole pharmacy team know what to do
It's now several years since TCAM first rolled out in the Southampton City and surrounding area around University Hospital Southampton (UHS). Since then TCAM has been widely adopted elsewhere across our CPSC area as well as nationally around the country.
In this time there has been much movement of pharmacists and teams, as is the norm in our professional practice, so in order to ensure knowledge and best practices are upheld. Please find below details of our TCAM webinar for you and your team to view to either learn for the first time or refresh and update their understanding.
TCAM is a most important platform that allows pharmacists to have meaningful conversations and interventions with people discharged from hospital that have had their medicines changed.
