'Transfer of Care Around Medicines (TCAM) service' - Delay of Southampton 'First 100 Patients' review event

UPDATE: This event has been temporarily postponed until the new year. A date will be announced in due course.

In the mean-time we encourage all community pharmacists to continue to look on PharmOutcomes for referred patients from UHS, to 'accept' them on the system and then contacting them to arrange a consultation. Finally, once each patient has been seen, please 'complete' the TCAM service on PharmOutcomes indicating significant activities. This is an opportunity for community pharmacy to gather more experience from TCAM prior to us hosting a rescheduled event.

The Transfer of Care Around Medicines (TCAM) service has now been active in Southampton since September. Around 100 patients have benefitted from the hospital discharge team working more closely with each community pharmacy, so it's now an ideal time to assess how things are going and to share best practices and behaviours amongst all contractors.
The meeting will provide:
- Recap of the service from UHS, including procedures & PharmOutcomes process (useful if you missed the initial launch event)**
- PharmOutcomes process and introduction to 'PharmAlarm'
- How is it for you ? Please come prepared to share your experiences
- Feedback from hospital and community perspectives
- Best practice sharing from UHS team and a highly active TCAM providing pharmacy - including local case study examples of valuable interventions achieved
- Update on data - how are we doing?
The meeting is coordinated and organised by UHS / LPC (CPSC) / WessexAHSN and will again have Deborah Crockford & Richard Buxton (CPSC), James Allen (UHS) and Clare Howard (Wessex AHSN) together with Kevin Noble (PharmOutcomes) presenting and able to answer questions during the evening.
It's important for all pharmacists from both in and around Southampton to book-in and attend this follow-up event to ensure we all provide a fantastic experience to all discharged patients using the service, as well as maximising the benefits from working collaboratively across our different healthcare sectors.To Book: Details will be available in the new year once date, time and venue have been confirmed.
