MOTIVE (Medicines Optimisation in Vulnerable Patients)

The MOTIVE service is a way of supporting patients recently discharged from hospital who have been identified with a need for further support with their medication. The aim of this service is to support patients with their medicines, thereby improving their adherence, reducing adverse effects and improving their quality of life.

All pharmacies are requested to attend this engagement evening to hear more about the launch of MOTIVE.

Selected patients that are discharged from hospital will once again be able to benefit from additional support provided when they go home to ensure that their medicines needs are fully catered for.

Mobile patients (those able to visit their community pharmacy) will receive a targeted MUR or NMS within their nominated community pharmacy, whilst housebound patients will receive a domiciliary review by a member of the medicines optimisation team who will liaise with the patient's nominated community pharmacist and GP.

Previous evaluation of the Pharmacy Reablement Service (2011 - 2014) showed that this form of support on the Isle of Wight was associated with fewer days in hospital, as well as being less likely to require readmission going forward.

Community pharmacists providing the previous Service also reported that it gave them a great sense of personal and professional satisfaction to see that they had directly contributed to patients being able to stay at home for longer.
