Annual Complaints Report 2024/25

It is a mandatory requirement for all community pharmacies, including distance selling pharmacies, to prepare an annual report on complaints by 31st March each year.

A request was emailed on the 9th April 2025 by the South East Commissioning Hub for all pharmacies to send the information to the pharmacy commissioning team.

Please do so as soon as possible using the MS Form link below.

If you are part of a pharmacy group or multiple, please check with your company manager and/or head office as some have been completed centrally.
