Joint LMC and LPC Medication Unavailable Form

New process to standardise and simplify medication unavailability messages between community pharmacies and GPs


Please see attached a revised request form to use when medication is unavailable.

The LMCs (Wessex for ourselves), Dorset and Swindon & Wiltshire LPCs, plus ourselves (CPSC) have all agreed that we will circulate this for use across the whole of our respective areas.


Wessex LMC are sending this communication to all their GP practices in their next newsletter, so use of the form should be expected when pharmacies start to use it over the coming weeks.

Please brief your team on its use. The idea is simple, so when a medication is unavailable then a patient bag label sticker and the respective medication label can be quickly attached to the form, before then sending to the respective surgery for the necessary action required to be instigated.
