A great new short video to support our increased involvement in patients safe discharge from hospital
It's time to meet Mo again!
This time the Wessex AHSN video shows Mo and how the transfer of care around medicines (TCAM) work can help those who have had changes to their medicines during a hospital stay.
The final version is now available on the Wessex AHSN website along with the original Mo video (Taking charge of your medicines with Mo). Wessex AHSN are happy for anyone to promote the videos to patients locally, via newsletters, TV screen etc. The comms team at the Wessex AHSN can also help if you have any specific requests.
