PQS: Information required to complete PCN domain declaration

Below is a list of all the PCN Leads, their Pharmacy Name and the Pharmacy ODS code. You will need this information when completing your PQS declaration during February if you are intending to claim for the Flu/ PCN domain.
When claiming for this domain you need to claim for the maximum points awarded for your banding e.g. Band 4 pharmacies will claim 10 points.

There will be a reconciliation of the payment made to pharmacies for this domain on 1 June 2022 when final data on the increase to the uptake of flu vaccination to patients aged 65 and over will be available. Data on the percentage of target population vaccinated by the PCN (GP Surgeries and Community Pharmacies) will not be available until after the PQS declaration window has closed.
Points will be awarded on a sliding scales between 80.1% - 86% of the 65yrs and over population vaccinated.
