'Transfer of Care Around Medicines (TCAM)' - Hampshire Hospitals go live

It's important now to brief the whole team following switch on of the TCAM launch

There's been an unfortunate delay in switch on of TCAM for Hampshire Hospitals due to a succession of IT problems in the period following the launch event in January 2019. I'm pleased to announce that these have now been worked through and the hospital pharmacy department team have been trained, so anticipate that referrals will now start to be received at community pharmacies via the PharmOutcomes platform.
As a reminder the TCAM process will be used at Andover, Basingstoke and the Winchester hospitals
Please find attached (below) further information from the hospital team that includes contact details; should you need to contact the team regarding any particular referral.

Advice from CPSC
- Get in the habit of checking PharmOutcomes at least every other day (if not every morning) – just make it part of daily opening/ start up
- Look out for meds that meet NMS requirements
- Don’t expect lots – even the top using pharmacy is only seeing 2-3 per week from the well established Southampton TCAM
